Congratulations on being selected by your league to manage this year’s All Stars! Thank you for continuing to volunteer your time for our young athletes. You are about to assume even more responsibility as you represent your league in the Ontario district 8 tournament and if you’re fortunate enough at Provincials, Canadians and possibly even the World Series.

An important early step in your All-Star season will be creating a Team Affidavit that provides proof that all players on your team are eligible by age, live or go to school inside league boundaries, and have played in the required number of games during the regular season. While this may seem like a lot of work and detail just to let the players play baseball/softball, please understand this is Little League International requirement of every tournament around the world. These details rules and validation process are in place to ensure everyone is playing on a level playing field and to the same guidelines. This can be quite time consuming and we suggest you start early to get this paperwork in place. 

The information below will help ease this process. Please know that while you may have a team representative work with all parents/guardians to collect the information needed, you and the league president are ultimately responsible for accuracy of the content. Your team will not be able to participate in the tournament until the team affidavit is approved by the District 8 district administrator. 

Tournament binder:


This is where to start. Get a 3 ring binder label the front and side with your league name and age group and then get several clear plastic page protector sheets. Put a tab on each of those sheets with a player's name on it. You will then in each of those clear sheet protectors have all the information for that player in this order:
- player verification form
- proof of age (usually birth certificate)
- proof of residence - either 3 documents or the school enrollment form
- waivers (if needed)
- medical release form
- photo release form
- travel release form

1. First sheet that goes in is the tournament checklist which you will complete to ensure you have all the documents you need for your players. tournament_verficiation_checklist.pdf

2. Once you complete the tournament affidavit and print it off this will go next in it's own sheet protector. Ensure pages are signed by manager, player agent and league president.

3. The signed map goes next showing all players are within the boundaries or that appropriate waivers are completed, ensure page 2 is signed.

4. Place the pitch count log sheet so it is separate in it's own slot. 

5. You will then put the players information packages in same order that is on the affidavit (alphabetical). 

6. At the back of the binder include the coaches' proof of diamond leader course. 

If this is a combined team, the approval letter should also be included just before the affidavit. 

This binder will travel with the team once playdowns start and will be asked for at the coaches' meeting. It will be signed off by the tournament director at each level of play and be requested prior to every game. The pitch count log will also be included in the binder and the managers need to sign off after each game. Please keep the binder safe as there is a lot of personal information in the binder.

This is a sample of what the affidavit looks like but it must be filled out online on the data center. 
Note there are areas however that once you complete it online and all the data is filled in you print it and there are areas for the league president, player agent and manager to sign on the affidavit and the map. Ensure those signatures are done prior to playdowns.

Once you have the affidavit and all player forms, please ensure your league presidents signs off on all forms and the affidavit.

Player verification form:

This is the form that all players must have filled out and you need to have this before completing the affidavit. 
This form once completed correctly and signed off, can stay with the player during the length of their little league career. So make sure you complete it properly and hang on to those binders.

Here is a how to guide for completing the player verification form to share with your parents. Please ensure all areas are filled out completely (don't leave dates or league ID blank) on the player verification form. Ensure you have a copy of the birth certificate included with the form as proof of age. Where most people get hung up is on the proof of residency. Note you can either provide 3 documents with your proof of address OR the school enrollment proof. The much simpler way is the school enrollment proof with the signed school enrollment form from the principle. If you are using the 3 documents note that you need a document from each of those 3 categories (easiest is driver's license, mortgage statement and utility bill) and note that the date on the document must be between Feb 1st last year to Jan 31st this year.

School enrollment form:
As mentioned above, if using school enrollment as proof of residence, this form must be signed by a school administrator. We do suggest this is done early before school is out for the summer.

Waivers:
 
A ll(d) waiver form is required when the player moves to a new address anytime after the end of the prior season or changes schools. The form is needed for the following:
a. Change of address within the same Little League boundary
b. Change of address outside of the Little League boundary but wants to continue to play with the same league.
c. Changed schools if used the school enrollment form as proof of residency.  
Or if there if the player is a sibling of a player who previously qualified for II(d) or II(a)

If a person had previously resided within the league boundaries for two years while serving that league as a dedicated manager, coach or member of the Local League Board of Directors for two years, his or her sons and/or daughters are eligible to try out and be selected by teams in that league (1) provided such service to the league from which the person has moved has continued, (2) subject to written agreement from the league within whoseboundaries they currently reside and (3) supported by a recommendation of the District Administrator, to Regional Director to the Charter Committee

Travel release forms:
This form must be included for each player based on the level of play. These forms are released by Little League Canada every season.

Passport form:
This form is completed by your manager and submitted if you go to provincials. The reason for this is that the winner of provincials goes on to Nationals and if there are flights to nationals, this is all arranged by Little League Canada but they need the passport information. So please ensure your players have a passport if travel is planned.

Combined Teams Forms:
If two leagues are combining players to make one team for all star play, a combined teams form needs to be completed. This is done on the data center and submitted to the district administrator for approval. 

Special games forms
Any teams that are playing games outside of their regular interlock schedule (ie tournaments or exhibition games) must fill out a special games form and submit it to the DA at a minimum of 4 days prior for approval which then will get approval from LL Canada. 

If the games are with only Little League teams, this form is filled out in the data center. Please also send the DA an email indicating it is awaiting their approval.
